We are delighted to have been asked to help with re-admission avoidance by providing home visits for patients who become unwell following discharge.

This will be a very short-term service, which we hope provides additional capacity as the paramedic visiting service is seeing several patients a day who have been recently discharged.

This is part of the work we are doing providing support during this time of exceptional demand for health and care services.

We will provide further updates – including a launch date for the new service – as soon as we are able.
