IN PICTURES: Hypertension screening on Ellesmere Green

Take a look at the most recent pictures of our pilot community hypertension screening service which has been out and about on Ellesmere Green carrying out a pop-up session in partnership with Foundry Primary Care Network (PCN).  

Despite the rain the community turn out was great and we carried out health checks which identified a number of patients with hypertension which wouldn’t otherwise have been found.

Primary Care Sheffield is piloting this new service to help three Primary Care Networks (PCNs) to identify more patients with high blood pressure (also known as hypertension) with the ambition of reducing health inequalities through prevention.

The pilot involves providing pop-up, appointment-free, blood pressure checks across Sheffield as well as education on self-care in local communities or workplaces. These sessions will be hosted in community locations and will target deprived areas, disadvantaged communities, and people who are less likely to engage regularly with health services.

Having blood pressure measures which are consistently above normal may result in a diagnosis of high blood pressure (or hypertension). The higher your blood pressure levels, the more risk you have for other health problems, such as heart disease, heart attack, or stroke. 

The pilot has been developed with Sevenhills PCN, Foundry PCN, and City PCN – and will be delivered initially in those areas.
