Primary Care Sheffield employs clinical pharmacists that are used in a number of Sheffield General Practices to deliver services to patients such as medication reviews, minor illness advice or chronic disease clinics. These pharmacists operate under the host practice’s governance and the information they collect is recorded on the host practice’s clinical system.

The information we hold
The host practice will keep records of any health care and treatment you have received from our pharmacists and they will abide by the privacy notices published by the host practice.
PCS will extract activity information only for the purpose of evaluation, payment and monitoring. Any information, we hold will be fully anonymised and will not contain your age, contact details or other personal information.
Your registered practice will retain details of your consultation within your existing GP clinical record.
Additional Information Sharing
We will not share your personal information outside our service. Information we do share will be aggregated and fully anonymised. Details of how your registered practice may use this information can be found in their fair processing notice or privacy notice which can be found on their website.
Data Retention
Your registered GP will retain access to entries we have made during your consultation and approach the management of your record in line with the Records Management NHS Code of Practice for Health and Social Care which sets the required standards of practice in the management of records for those who work within or under contract to NHS organisations in England, based on current legal requirements and professional best practice. PCS will not retain your personal information.
